Un cas de sarcome stromal de l’endomètre chez une femme malgache de 20 ans

Résumé
Le sarcome stromal endométrial est rare. Nous en rapportons un cas inhabituel observé chez une femme malgache de 20 ans. Elle présentait une volumineuse masse pelvienne douloureuse découverte depuis une semaine. L’échographie montrait une masse utérine hétérogène, vasculaire au Doppler, faisant 10x15x10cm. En six jours, cette tuméfaction augmentait rapidement de volume avec apparition d’un syndrome péritonéal motivant une laparotomie d’urgence. L’exploration chirurgicale montrait une volumineuse tumeur utérine rompue dans le péritoine. L’histologie concluait en un sarcome stromal endométrial de bas grade. Les particularités de la prise en charge sont discutées à travers une revue de la littérature.

Endometrial stromal sarcoma. Report of a case in a 20 year-old Malagasy woman
Summary
Endometrial stromal sarcoma is an uncommon disease. Authors report herein an unusual case observed in a 20 year-old Malagasy women. She had a medical history of painful voluminous pelvic mass discovered since one week. Ultrasonography showed heterogeneous uterine mass, vascular on Doppler, with 10x15x10cm dimensions. After six days, the tumour dimensions quickly increased and peritoneal symptoms appeared. Laparotomy was performed urgently and showed a huge uterine tumour broke in peritoneum. Histological examination revealed low grade endometrial stromal sarcoma. Diagnosis and treatment are discussed with a review of the literature.
